- Below are Release Forms, Par-Q Forms and Health Questionnaire Forms that must be filled out before we can start training. If possible, please email them back to me a couple days before our initial assessment session so that I can thoroughly read through them beforehand. If you happen to have technical issues signing or filling them out electronically, this iOS/Mac app works great (https://pdfexpert.com); OR bring a filled out physical copy to our session and we will go through them then.

- Bring a blank notebook (e.g., https://amzn.to/2Vnu39m ) and pen/pencil with you (You can also use your phone if you’d prefer an electronic note taking option). This will be your dedicated health/workout journal that you will want to have with you every time you workout. You’ll use it to log your workouts, accurately track your ongoing progress, workout pitfalls, comments/thoughts, injuries/ailments, etc).
- Bring water to stay hydrated.
- Wear “flat, solid-sole” weightlifting shoes over soft-sole/running shoes.
- Converse/Chuck Taylor’s (or something similar “sole-wise”) can work for now if you have them.
- A “solid-sole” weight lifting shoe is the best option long term.

During this session, we will go through your filled out release forms and health questionnaire forms, answer any health/exercise/training questions that you might have, and go through an initial assessment workout so I can properly assess your current health and fitness level. 🙂

IMPORTANT
If you ever need to cancel or reschedule a scheduled session (scheduling conflict, starting to feel ill, etc), please let me know “at least” 24 hours in advance of your scheduled session so that you do not get charged for the allotted time. I’d rather you cancel days in advance (and reschedule the day of) than to cancel late and get charged for it.
