Coaching Agreement
This Coaching Agreement (“Agreement”) is entered into between BodyFit LA, LLC and the client (“you”). By purchasing or participating in any of our services, you agree to the following:
1. Purpose of Coaching
Our coaching is intended to educate, support, and empower you to improve your health and lifestyle. It does not replace medical advice or treatment.
2. Scope of Services
- Personalized health coaching, fitness training, and nutrition planning
- Guidance based on intake forms, daily tracking, and functional lab data (if applicable)
- Email/video support and weekly feedback as specified in your program
3. Client Responsibilities
- Provide accurate and complete information in all forms
- Communicate openly and consistently
- Follow recommended plans to the best of your ability
- Notify us of changes to your health, schedule, or goals
4. Confidentiality
All shared information will be treated with strict confidentiality and used solely for coaching purposes, except when disclosure is required by law.
5. Payment Terms
All services must be paid in full before coaching begins. No refunds will be provided unless otherwise agreed upon in writing. Service charges may apply based on payment method.
6. Scheduling & Missed Sessions
24-hour notice is required for rescheduling or canceling sessions. Missed sessions may be forfeited without makeup or refund.
7. Limitation of Liability
You agree that BodyFit LA, LLC is not liable for any direct or indirect harm or injury that may result from participation in any coaching, exercise, or nutrition plan.
8. Termination
Either party may terminate this agreement at any time with written notice. Services rendered prior to termination are non-refundable.
By continuing with services, you affirm that you understand and agree to the terms outlined in this Coaching Agreement.
